The Transportation Safety Board of Canada is out with it’s report into the deadly May 2013 Ornge helicopter in Moosonee.

Pilot Don Filliter of Skead (ski-eed), along with another pilot and two paramedics died when the air ambulance helicopter they were in crashed shortly after take-off.

The T-S-B says Ornge did not have sufficient, experienced resources in place to effectively manage safety and the tragic outcome was that an experienced flight crew was not operationally ready to face the challenging conditions on the night of the flight.

The board has issued 14 recommendations in three areas including regulatory oversight and aircraft equipment.

Ornge says it has made many changes since that crash, including taking that model helicopter out of service and hiring new staff with extensive experience.
